Same day appointments are available. To schedule an appointment, please call (941) 752-2882 during regular business hours.

New patients

New patients are asked to arrive 30 minutes early to complete paperwork. Our new patient forms are available on our Patient Forms page. You may print, complete and bring the forms with you to your appointment. Please also ensure that you have with you at your appointment the following:

  • List of all medications you are currently taking
  • Any recent copies of x-rays and/or lab results
  • Photo identification
  • Insurance card(s)


Payment is due at the time of service. We accept cash, checks, debit and all major credit cards.


We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address or insurance as soon as it occurs. Please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill requests

Refill requests are handled electronically in conjunction with your preferred pharmacy. Please ask your pharmacy to contact your physician for your medication refills. If you need to speak with one of our medical office staff about your prescription please call (941) 752-2882.

After-hours & emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (941) 752-2882 and our answering service will assist you.
